INSTRUCTIONS FOR COMPLETING THE CLAIM FORM

If you believe you are an eligible Settlement Class Member and you wish to apply for a Settlement benefit, you must complete and submit a Claim Form. Please read the full Class Notice available here carefully before completing a Claim Form.

If you submit your Claim Form online, you must do so on or before January 21, 2021. If you are mailing your Claim Form, you must do so via first-class United States Mail, and it must be postmarked no later than January 21, 2021.

If you made purchases totaling less than $150.00 or you have no proof of purchase, regardless of the amount spent, you may elect to receive 1 Coach outlet Voucher worth up to $20.00, or $5.00 cash. If you have proof of purchases totaling $150.00 or more, you may elect to receive 2 Coach outlet Vouchers, together worth up to $40.00, or $10.00 cash.

CLAIM FORM REMINDER CHECKLIST

Before submitting this Claim Form, please make sure you:

  1. Complete all fields in Section A (Name and Contact Information) of this Claim Form.
  2. List all of the Coach Outlet Merchandise offered at a discount from a “MFSRP” purchased in Section B of this Claim Form.
  3. YOU MUST sign the Attestation under penalty of perjury in Section C of this Claim Form.
  4. Include copies of your Proof of Purchase documentation that you are submitting in support of your Claim Form. Do not send original documents.

SECTION B: COACH OUTLET MERCHANDISE PURCHASE VERIFICATION

Provide the following information regarding purchases you made during the Class Period of Merchandise offered at a discount from a “MFSRP” at Coach-branded outlet stores in the United States, and indicate the purchases for which you are attaching Proof of Purchase documentation. Proof of Purchase means receipts or other evidence establishing that you purchased a specific quantity of Merchandise offered at a discount from a “MFSRP” at a specific price, on a specific date, from a specific location, during the Class Period.

There is no limit for Merchandise offered at a discount from a “MFSRP”. Limit 1 Claim Form per household.
